I liked the design by Andreas Gohr and changed a few things to my taste:
• chamfers on the mounts • one mount with hex nut trap, so now two different mount pieces exist • only 1 mm gap between the mounts that disappears if screwed tight
BOM is the same:
• two 608Z skateboard bearings • one M8x60mm screw • one M8 nut • one M8 washer • one M5x35mm screw • one M5 nut
